Passport No. (Write Her Canadian PP No. ) do hereby solemnly affirm and

declare as under :

1. The place of birth, Boriwali IND which is mentioned on my Canadian Passport ( Write her Canadian PP no.) and the place of birth, Borivali Mumbai which is mentioned on my Indian Passport ( Write her Indian PP no. ) are in fact one and exactly the same.

2. This is a true statement.

Deponent

( Your Wife's Signature in Blue Ink )

( Your Wife's Full Name below her signature )

Somewhere on the above AFFIDAVIT the lawyer or Commissioner of Oath will stamp his seal & manually sign it.

BLS will take this original AFFIDAVIT from you, so take a few photocopies before hand if you want them for your record.

In that AFFIDAVIT you must also have a place and date on which it was written. AND

It is always signed with BLACK INK. NOT BLUE.

Really ? Always Black ?? Stop spreading your misinformation.

Blue ink is preferred in India as its easy to distinguish between the original & photocopy. He is going to BLS & the documents are going to Indian embassy after all.

Not that if he uses black ink it will be the end of the world. But there is NO such rule that it should always be signed with BLACK ink. Either will do, with the preference being BLUE.

Place & date is commonsense when signing most documents. OP will figure it out himself. I just gave him a general guideline.

When the lawyer signs & stamps the affidavit, he puts the date & place anyways.

Thanks Pendse & Full house . I prepared affidavit as mentioned by you and get signed+ stamped by commissioner of oath . About signature he advised my wife to sign in blue , he mentioned that there is no harm to sign in black but to its easy to distinguish if she sign in blue .

About documents for OCI .
I sent email to ministry of foreign affairs and then follow up with CGI Vancouver and BLS -Surrey office and get email confirmation from them that only Indian passport to be submitted in original ( for surrender ) and Landing paper in original or notorized copy . All other docs should be self attested photo copy . I have email confirmation from CGI & BLS so I am not going to send all other docs in original nor notorized . I will just attach that email print for clarification .

Here is the link to BLS, Check list for submission of OCI Application.

http://www.blsindia-canada.com/CanadaVisaCCT/BLS-CAN09Toronto-NewOCI-Checklist.pdf

See if all of the boxes are ticked. If they are then, the application that you submitted is COMPLETE.
