Incorrect credit card statements!

Dear CD's,
This problem started a 2 months back with my PC Mastercard credit card statement. In that statement, I could find random credit's to my credit card statements and the same amount of debits to my statements (those credits in my credit card statement reflected were always for some transactions that had occured within a period of 3 years before). Since my debits equaled my credits, I did not bother much.
Today, I received 2 credit card statements from diff. credit card companies. One was PC master card in which i had a credit amount of 100 $ and immediatelty a corresponding $100 debit. Also I have never been creited ever with PC points for this portion of credit / debit transactions.
The second one was a AMEX master card which I had signed for in COSTCO but had never even used it once. A few weeks before, they had debited it with an automatic amount for the annual membership of Costco. Since SS had informed me before a year that i could cancel the member ship of Costco at any time, I cancelled my Costco membership and the representative swiped my card to give me a credit. In the statement that i received today, I found that i was debited and then credited for my annual membership cost (so the total amount to be paid is $0) and then I was CREDITED for $300. I donot know how this happened as I had not even activated my card and the only time someone ever swiped was at COSTCO to put credit in it for the cancellation of my annual Costco membership.
Am I in the starting stage of an Identity theft process? I am mostly not careful with my personal information and throw many credit cards/ bank statement straight into the garbage once i get them in my mail box? What should can i further do to safeguard myself apart from the normal stuff like shredding credit cards/ bank statements. Has stuff like this happened to any other CD's?
Your view would be highly appriciated and would help me to decide on my next course of action.

I don't think u have any severe problems that cannot be solved by a single call to the number given on the back of the respective cards...

If you are concerned about identity theft, the first step is to put an alert on your file in all 3 credit bueraus.

Identity Theft can be checked from the following companies:

http://www.transunion.ca

http://www.equifax.com/EFX_Canada/

One of hem give free report if you fax details to them. Anyway pls visit these sites and see that all credits reported are approved by you. If not contact them to dispute the transaction history.

It is important to keep the credit clean here and it is easy for criminals to get personal info from many places including your garbage you throw out. Pls do not throw credit card offers that come by mail. This may have your personal info.
