I delivered my daughter last year while on TN in Los Angeles...I had private insurance too from Blue cross of CA HMO plan. Total insurance cost was around $36K for delivery only plus each time i went before delivery for contractions $5000..Total cost I paid from my pocket was around $2000 and $25 copay each time i visited the doctor for checkup. Rest was paid by insurance....
I had this insurance since Jan 2006 until Jan 2009...You need to have insurance effective atleast for 6 months before pregnancy if you want the insurance to cover your pregnancy . This is mandatory. I also had my company insurance effective..
This is not mandatory for everyone. Your company's insurance benefits seem to be crapy (if this mandatory rule is from your company's insurance) - because almost every employer provided health insurance covers all pre-existing conditions (including pregnancy). The case that you described applies only if you buy your own health insurance.

Another update --- a friend of mine from the Middle East visited US and got both his children delivered in the US .. while on a visit visa each time ...
They gave the reason/s for visit as touring .... but paid the delivery expenses which came to around $10K for delivery alone .
So it is permissible and workable .
