In Toronto today morning I saw some availability so you really need to check real time atleast on weekdays.
on another note you pay $9 per account per reservation and not for individual applicant.As a matter of fact you can also add family member after you confirm your reservation.Check the FAQ section.

Once you reserve the appointment you will get all documents like DS156 etc part of your confiimation package.

10) What fees do I need to pay?
For all non-immigrant visas there is a non refundable U.S. $100.00 visa application fee. Effective July 1, 2007, individuals applying for U.S. visas at all Consulates in Canada must first deposit the application fee to a ScotiaBank branch located in Canada. The deposit slip for your visa application fee consists of 3 pages and is available at http://www.amcits.com/deposit. A separate set of deposit slips is required for each applicant. Take all three pages of the deposit slip(s) to any branch of ScotiaBank, and pay the non-refundable U.S. visa application fee of US$100.00. The bank teller will stamp all three copies of your receipt and should return two copies to you. You must submit two validated deposit receipts with your visa application.

For appointments, check out the website on Monday mornings around 8 AM for sure. I think that is when you have the highest chance of getting an opening for interview. Three of my friends got it at that time. One got a slot within 5 days even.
