IELTS results are valid for two years only, I have to apply from Islamabad, 64months waiting time in Simplified process, after 64 months do I have to take IELTS again or what ? If I wait for the 64 months and do not take IELTS , how can I claim my language proficiency in application form. Also I Have 5 years USA Visit Visa, can I apply from USA for Permanent Residence.

No, once you submit IELTS result within that 2 years of period then it should be fine ...

ya..i think its better to apply from USA, process will be fast, but i dont you can apply from USA or not, if possible then good, if you are applying from isalamabad then again you have to go for IELTS. If you are married you req two 5 and two 7, if bachlor they you need all 7 7 7 7
