The details provided here under are 'also seen' on the cheque leaves that have the Magnetic Ink Character Recognition provided to you or me on each bank account with them.
When you approach a bank they will provide you all of these for an ELECTRONIC FUND TRANSFER.
Each Bank has a ABA number and also a SWIFT code number walk in and find out. I have them for my account here with them.

Depending on where you are getting the funds from, either as a gift or from a sale, you will need the help of a Chartered Accountant. If it is a gift, then ask your friend to go to the bank and request them to send it to your bank here in Canada. I am sure you would have given most of the details to him that they require at that end.
The above form that you have in the link needs to be filled in by who ever is sending the money to you.
The Reserve bank of India is currently very liberal in allowing the dispatch of money out of India. So ask your friend to talk with the manager of the bank and see if it needs this form filled for release of the Foreign Exchange. If he insists then he will have to fill it prior to his giving you the Foreign Exchange.
If they do have free transfer facilities between the banks, then avail it.
