Name change- shortening last name in Ontario

I would like to know if I want to shorten my last name, does it require birth certificate? For example: from Anuradha Mohankumar to Anuradha Mohan.

If you are going in for an official change of name you will have to provide a copy of your birth certificate along with other documents (citizenship certificate/record of landing/all other name change documents (marriage certificate)

Your name is not as complicated as mine. So, I sometimes shorten it to my initials which ends up as SS (you know... like the Gestapo). Depending on how sensitive people are to political correctness, I either get a frown or a good laugh.
