It is always better to transfer the PIO/OCI details on the new passport for children below 16 and Adults above 60 (as said in the website).
This will solve hassles at the Indian airport in case a guy who knows the law is your immigration officer.
Say if he has a print out of the rules, what would you do? Offer a bribe?

While it is true that no one has reported any problems by carrying old and new passports together with PIO card, yet I do not want to take any chances, so I will get my daughter's new passport endorsed on the PIO card when the time comes.

My son has a PIO card and recently got his new CDN passport. Is it required to get the new passport # in the PIO card or has anyone travelled recently with the old and new passport with PIO card.
if it is required do we go the Indian consulate in Toronto or BLS? Any recent experiences?

As it has been stated, Indian officials have not asked for endorsement in the PIO card/OCI for kids below 18 years. So it should not pose a problem.
My experience with landing in Tier 2 cities is that they donot know the rule and you may escape. Even if you get caught, it will be upto the discretion of the chief immigration officer with whom you may have to haggle a bit.
But then, it would be better if you get the endorsement as it can save you from any possible panic attacks.

As mentioned by dan above, the Indian consulate says that the new Passport endorsement HAS to be done on PIO card.
But this position of CGI, I think , is partly misleading and is a means for boosting revenue earnings.
The competent Authority to decide on such issues is Bureau of Immigration, India ( They are the one who will grant admission to PIO person, not CGI or other consulate generals)
The Bureau of Immigration clearly states
"Foreigners holding PIO cards-
The foreigners holding PIO cards can gain entry into India if valid PIO card is presented along with the valid new foreign passport issued in lieu of the passport endorsed on the PIO card.
However the foreigners are advised to have necessary endorsement of the new passport from the competent authority on their PIO cards to avoid any inconvenience"
So, the endorsement is recommended, but NOT mandatory and Not a means for denying admission.
