Hello
My wife is already canadian citizen and holds OCI visa/card.
We have decided to change her maiden name to my last name.
I understand I need to change drivers license, OHIP, SIN, Canadian passport first.
What would Indian consulate require from her?
Marriage certificate we have is issued in India.
Any other documents or affidavit?
We do not want to do legal name change.

The first one should be CAN CITIZENSHIP CARD & CERT has to be updated. They won't change maiden or original name but WILL ADD UP MARRITAL NAME on card and cert. (pls keep in mind, that it will take little longer time.)
She will be allowed to use either name in future incase she wants to use maiden name for any reason.
Second, obtain passport under marrital name.
Third, Indian Consulate requires her to apply for OCI name change under Misc. Service and print prepare 2sets as per OCI Check list, 2 new photos too.
*Do not forget to apply with new passport number and her marrital name.
Marriage crt is OK from India, but it has to in English or translated notorized copy will required along with original.
**Citizen ship, OHIP, SIN card all will gladly accept marriage cert and change the name to marrital name BUT Indian consulate will ask for official name change for sure-WHICH IS NOT NECESSARY or IMPORTANT since your Marriage Cert, CAN citizenship and PPT has confirmation, just ask them to give you the requirement in writing - you will take it to media, then they will listen you and change it. Simple thing Indian consulate has different rules for each individual.

So what i understand form your reply is -
Add marital name to Canadian citizenship card and Certificate. Obtain canadian passport in marrital name.
Once all the above is done. I apply using OCI miscellaneous form.
Quick quetion - will Indian consulate ask for legal name change doc? we do not want to get legal name change done.
Do we get a new OCI sticker and card with maritial name?

Your marriage certificate was issued in India, so you may want to clarify with VFS if the below applies to OCI Miscellaneous services.
http://www.cgitoronto.ca/Passport/passpMisc.html states that for miscellaneous consular services, marriage certificates must be attested by Consular Section, MEA, Delhi.
It depend on who you are dealing with. Most cases yes, they ask for legal name change doc. They will ask for legal name change doc but it is not that every one give them or must give them. They know that large number of married women don't go for legal name change, they just use marriage cert to change name and it is widely acceptable.
This is what exactly in OCI Check list:
Interpritetion is different for each applicant:
11. Change of name documents: In case the name of applicant mentioned in the Indian passport, the Record of Landing and the Canadian (or Foreign) Passports differ (i.e. whole or a part of the name / surname has been added/ dropped / changed,) Change of name certificate, sworn affidavit, deed poll attested by competent authorities and clippings from newspapers in India and Canada would be required to be submitted. In case of change of surname of women after marriage, a marriage certificate issued by the Registrar of Marriages’ office would be acceptable.
*Simplest thing will be sworn affidavit will be asked to submit.
(In some cases Consular do accepts hand written note or request.)
Formal affidavit may cost you $25-30 by lawyer or (free at Neighbourhood community service desk-it may take 2-3 days).
OCI card and U sticker will be replaced with her marrital name and passport number after successfully approval.
