As long as you make it clear from the beginning and stick to the same name that you want them to be appear on passport it should be fine (of course u need to have additional supporting documents attached along with the first submission) - cic mostly wants clear picture from begining - they are well aware of confuse name situation - it is very common for them - just make it clear but very important is , not to confuse ur self first.
Sir name is necessary in all documents – so u must add it. – do not leave it blank
Note: is it possible for your father to make new passport with sir name ? ( by fast forwarding process)

I am confused - My father's passport doesn't have sur name ... And I know that the PR Card if issued will be based on the Indian Passport ..... So I have not added the Surname in the form except at the place where it asks if you have used any other name ....
What happens if we leave the surname blank as of now and let it continue till the time the PR is issued and then in the future if and when they would apply for citizensip here as well as Passport here , we can request for a change in name if desired ......
Could some one tell me if there can be some issue in leaving the surname blank in the form as it is blank in the Passport as well ??
