Hi Vijaya,
I can understand your anxiety as the particular answer you are looking may not be on the web site. However, the immigration officer would not likely ask you if you plan to return immediately back to US and you do not necessarily have to say it to him/her. Sometime they say 'silence is golden'.
You would have to provide an address in Canada wherein they can send your PR and the same address you would have to provide them for the health authorities for medical monitoring etc. So, please make sure that you get all your communications/mail etc. even if you are in US and you can tell them later that you have gone back to US and would be reporting for medical surveillance after returning and I believe there should be no problem. Or, they may give you the option of taking certain tests in US and forward them the result etc. (but I am not sure and you can call them and confirm)

(Indeed, I did reply to your query because I have been thro a similar process).
Since you had been thru similar process, do you remember anything about IMM 535.
It has been specified in Remark - IMM 535 Attached. But I didn't receive that. I am not sure whether immigration authority at point of entry would provide that form or it should be along with the confirmation of permanent residence documents.
Please advice. Your suggestions are appreciated.

That is what I asked? You did get a "genuine" reply from akhan, but you did not appear any less confused.

The URL quoted by akhan talks about that in no unspecific terms, that, you are required to report to a designated practitioner within 30 days of your landing and 4 working days are part of that. I do not understand what was unclear if you indeed read the document?

It has been a long time, and I am not sure I remember.
From what I understand, the form should have been given to you by the immigration office.
Here(please scroll down) is a specimen copy of the form in question.

If you notice, the first of the three copies seems to be marked for "ORIGINATING VISA / IMMIGRATION OFFICE". This appears to imply that the form originates from the immigration office that issued your visa.
I suggest you call up one of the CIC info centers and get clarifications. If you cannot contact them from the US, maybe someone in Canada can get the required info for you.
