My parents has mutiple entry visa - on visit since Apr'2014-Oct2014.
We would like extend for another 6 months.
do we have to buy insurance for extension submission ?
cic tells parents /grandparents insurance for 2 years visa which is called extended stay- but no details on 6 months extension.

Insurance is a means by which everyone defers their expenses and limits their costs. What I mean by this is, you pay a very small sum as a premium, and cover those expenses in case there is an illness and the cure for which will cost you tons of money.
Is it a must? NO. It is optional in some cases. For Super Visa, as the time spent here is a very long period, the probabilities of a serious illness occurring during the period of stay increases and the Canadian Government does not want an Immigrant to go broke and leave the country in these extreme circumstances.
The sum you pay as a premium for BOTH your Parent's Insurance, will not even pay for ONE Days stay in a hospital.
So, you be the judge. To insure or to not insure, is your decision.
