401K and Canadian tax

Folks
I Moved to Canada from US in Jan of 2005. I encashed my 401K ( retirement savings ) in US last year. I have a statement from JP Morgan ( my 401k fund manager ) showing my total amount and tax that was deducted in US before sending the check to me here. Question is
1. how do I file tax on this or where do I show this money while filing tax here.
2. Also how do I calculate my income on this as I've not put money in 401k since I moved to canada and I do not have any statement from JP Morgan for 2005 or 2006 as no activity happened ...

Any idea ??
