I am new to this forum. So, if any other thread is open for CPP, please guide me accordingly.
Currently, I am PR, since four years and awaiting for Citizenship test.
I am employed with prestigious employer of Canada for four years and contributed in CPP (Canada Pension Plan). Employer also contributed in pension plan and provided pension statement (Private Employer's Pension plan).
I am 50 and due to health reason, I have to move back to India in next 6-7 months. I don't want to move back but, I have to move back.
I have also my maximum possible RRSP contribution.
Am I eligible for CPP?
How it works?
Is it possible to apply for it?
Are there any other benefits?
If I am not eligible, how can I get my contributed CPP from Govt of Canada?
Basically, I have no clue, how it works... please guide maximum.

couple years back, I had suggested on CD that we Indians are prone to die very early as has been proved statistically and I was ridiculed. U r going back due to health reason. Anyway, following is the link and Info.

Just to draw your attention that if your health condition is such that you cannot work, then it is better to go on disability (which many Canadians do and many a times FAKE it too).
How do I qualify?
You qualify for a CPP retirement pension if you worked, you have made at least one valid contribution (payment) to the Canada Pension Plan, and you are at least 60 years old.
Your retirement pension does not start automatically. You must apply for it.
Exception
If you are receiving a CPP disability benefit and you turn 65, your disability benefit automatically changes to a retirement pension. See “What happens to my disability benefit when I turn 65?” for more information.
Do I have to stop work to receive my retirement pension?
You do not have to stop working to receive your retirement pension.
How does my age affect the amount of my pension?
Although the CPP retirement pension was originally intended to start the month after your 65th birthday, you can begin receiving your CPP retirement pension anytime after age 60. Your monthly payment is smaller if you begin receiving it before age 65, and larger if you take it after age 65.
If you start your pension before age 65: The CPP reduces your pension amount by a set percentage for each month that you take it before age 65, calculated from the time you begin receiving your pension. For example, if you had taken your pension at age 60 in 2011 (60 months before age 65), the reduction would have been 0.5% per month, for a maximum reduction of 30%.
From 2012 to 2016, this early pension reduction will gradually increase from 0.52% to 0.6% per month. This means that if you start receiving your CPP pension in 2016 at age 60, your pension amount will be 36% less than it would have been had you taken it at age 65.
This adjustment is permanent—if you choose to start your pension before age 65, your reduced pension amount does not increase when you reach age 65.
If you start your CPP retirement pension at age 65: You will get the unadjusted pension amount you are eligible to receive.
If you start your pension after age 65: The CPP increases your pension amount by a set percentage for each month that you delay receiving it after age 65, up to age 70.
From 2011 to 2013, this late pension increase will gradually rise from 0.57% to 0.7% per month. This means that, if you start receiving your CPP retirement pension in 2013 at the age of 70 (60 months after age 65), your pension amount will be 42% more than it would have been if you had taken it at age 65.

So, it means that I have to wait for 60 years (earliest), till I receive my pension from CPP. Just wondering, can I get CPP earlier than 60 years of age?
Only Citizens get CPP, right? OR even the PR, who have contributed for CPP, also gets CPP?
Now, following is new aspect for me....
What is disability pension? If there is genuine health reasons, can PR/Citizen gets disability pension? From where one can get medical certificate/document to prove disability?

I think there is nothing like disability pension but there is disability benefit which is provided by your employer's insurance company.
Canadian laws have been drafted by the most crooked and sick minds and hence you have to be really paying attention to every word and how the same has been defined for a PARTICULAR ACT. For example, the term "disability" has different meaning from point of view of a doctor, patient, under CPP, under Incometax Act, under disabled parking rules etc.
My understanding is that if you are having such a medical condition that you cannot perform your normal job function and the company cannot make alternate arrangement/ provide work which can be carried out even with medical condition then you may be eligible for disability benefits till you are fit to work again. Usually, your pension (company) and CPP would perhaps kick in early without penalty. I have heard about number of people whi have lived off their life on disability benefits.
Just cautioning you that before you make a move back to India, explore this option. MMS/Sonia has collected billions of dollars in their coffers and you would not get a dime from GOI.

I will explore early CPP, if possible before 60 years of age.
Also, if my medical condition will fit into disablity condition, then only i may apply for disability benefits.
