Yeah, you usually see a specialist after the first trimester, but again, it depends on your family physician.
Like Raj mentioned, you have to see a family physician who will then recommend you a specialist. In the second trimester, you usually will have appts. with the obs. once every month. During the third trimester, the obs. may want to see your wife bi or tri weekly.

Then, Perhaps it works a bit differently in GTA or Ontario, Or maybe its gynec or physician dependant.
In Montreal, We never needed no referral to see our gynec.

If your wife has a more than three months left than there are many charitable clinic in ontario which check regularly to your wife without charging.
But if you have less than three month than you become ready to pay hospital charges and extra medial test charges.
(But one more adwise if you come here and your wife need to hospitalise immidiatly don't give your SIN no , just say them i am new immigrant and i have just applied for that so iif might be helpful if you are not able to pay it doesn't iffect to your credit.)
