1. When you apply for single entry visitor visa; do you get the passport back from embassy in India with 30 day or month visa already printed on your passport OR the immigration office in Canada determine your lenght of stay once you land?
2. What kind of insurance is recommended when you come to Canada on visitor visa? Are there any recommendations from CD's here?

The validity of the visa (visa expiry date) indicates that the visa holder can enter Canada by that date.
Allowed duration of stay will be notified at the port of entry in Canada. If nothing is stamped during entry, one is allowed to stay up to 6 months.

You shouldn't and there is nothing in the application form that allows you to say that. Just so you understand, even if one gets a 5 year visa, that person can only stay up to 6 months at a time (unless formal/official extension of stay is applied for after arrival into Canada).
The best recourse is to say you need to stay for max. of 6 months. When the 6th month approaches, you can contact them again for the extension of another 6 months. This could be done in special circumstances and make sure that you give a valid reason for the extension.
However, I have seen the visas get extended for another 6 months in most of the cases. I think the maximum is 1 year that you could stay. After that you need to go back to your country of origin, and apply again in about 3-4 months. One of my friend's parents do this all the time. They come here on 6 month's visa, get it extended for another 6 months, go back to India and stay there for 4-5 months and then come back here again for another year. Fun, huh?
Check your options carefully.
