Actually, it looks like for the larger ($100,000+) policies (which you would want in order to to cover the possibility of a longer hospital stay to recover from stroke or heart attack etc.), the Travel Guard policy now offered by http://www.bestquotetravelinsurance.ca/visitors-to-canada-insurance would be the lowest cost policy. Some of the secondary coverage like emergency dental and out-of-pocket expense coverage is lower than GMS - but the savings is significant, and the policy does cover the major medical expenses that visitors really need to be concerned with.
This new Travel Guard policy - it wasn't offered there before - is the lowest cost visitors to Canada policy you'll find from any Canadian insurance company. For the $50,000 and below coverage amounts, Travel Guard is still cheap, but the lack of benefits means GMS or TIC is probably the better bet - until they change their rates!
Also there was a comment about getting insurance for over age 69:
Travel Guard's policy is only available up to age 69 and for no more than 183 days. So visitors older than 69 can get a quote for their GMS (best bet)coverage here: http://www.bestquotetravelinsurance.ca/visitors-to-canada-insurance and visitors planning trips longer than 183 days should think of using two policies - buy the Travel Guard policy for the first 183 days, and then extend insurance by buying another company's (GMS TIC or manulife depending on pre-existing conditions) for the remainder of their stay.
