Hello friends

I am required to submit a medical form to LIC in India for one of my old policies I had with them
They require a medical form submitted attested by a doctor here

Any one with experience having filled these before? I need to go to a doctor who can sign the form for me

Any input on any doctor who has done or does this would be of great help

You can contact your family doctor or Walk in clinic.

I did this in 2011 at a walk in clinic at Dundat st. W and Queensaway.

Process was simple. Gave the form to the office manager. She filled in everything.
Doctor came and did some tests and signed the form and left.

Paid the office manager. She gave a bill. This may be tax deductible.
