After that go to your FB Account - Privacy Settings - Apps & Websites -

Under Apps you use you will find Remove Unwanted or Spammy Apps .... Click on the red cross and you will be taken to the apps you use which should feature the Mazda App. .... Click on the cross at the extreme right of the App and it shall be deleted from your account ..
