Always travel by Emirates airbus if u want smooth travelling for seniors. Their services are better and connecting flight is in Dubai and most of the time it's on the same floor and they guide u well. I find it comfortable to travel with one stop i.e Dubai with my small children. happy journey to all who are travelling.

Under all cases , before they get off the flight they should ask the crew about the next wheel chair .When they are about to leave the wheel chair , they should ask the wheel chair for the next directions .
The wheel chair people are experienced and nice and are aware of the ground procedures and processes. They should not only guide you through Customs / Immigration but also be able to direct you where to get the porter from.
Its advisable to carry a print of the itinerary which can be shown to anyone at any time to find directions. Worst case ask for the nearest information desk from any uniformed person.... but do not try to go on your own -- wait for the wheel chair person where applicable ...

The link that Adam has provided mentions that the link train is wheelchair accessible so I would imagine that the wheel chair assistant would accompany you .
For checking out the luggage and checking it back in ... use a porter for worry free even if costs a little.
Most likely Customs and Immigration & baggage check out would take place @ Toronto and then another check in would take place at the next terminal .... but please read the link carefully.
