I will be moving to US next month, so I need to know the tips and tricks from you guys.
1. How should I handle my address?
2. Should I keep the Bank accounts open?
3. What about Health Card and Driving License?
4. Do I need to inform CCRA about my move? If not, Do I need to inform them new forwarding address?
5. What about child benefits?
6. I got a consulting firm. Can I keep it running?
I don't want to loose the PR and I am very much wants to apply for Canada Citizenship in the coming years. (already stayed here 1 year)

Usually people give friends address. You can inform post canada about your new address and they forward all emails to that address. The charges are about $35 for 6 months. I am not sure about forwarding to US address.

If it doesn't cost you anything - why not? If you are leaving Canada forever then it is better to close it.

You don't need to inform CCRA about anything. Why? At the time to citizenship application you have to mention your stay outside Canada. As you are moving to US legally, you have to show your absense.
Not sure about child benefit. It is better to inform them that your child is not stuying in Canada and let them decide what to do. I know few people who returned their kids to India but still getting benefits!
You can keep the company running - in fact it is better to claim your ties with Canada while you were away!

Eventhough there is a tax treaty between US and Canada, I still have to file the Canadian taxes (with zero income). Am I right?

I think somewhere massood answered to someone that you have to file your tax return at one country only - may be Canada or US. You can ask him directly.
