Does applying for US Visitor visa require only DS-156 form, or DS-157 as well? The requirements only mention the DS-156 as in following:
B2: Visitor for Pleasure
DS-156 (application form).
Valid Passport (at least 6 months).
Proof of legal status in Canada: original or certified copy.
One passport-size photo.
Older passports containing previous visas.
Documents substantiating previous legal status in U.S.
Non-refundable U.S. Visa Application Fee
Reciprocity fee (if applicable).
Proof of sufficient funds to cover all expenses while in the U.S.
Proof of ties/residence in a country outside the U.S. which have no intentions of abandoning.
Evidence which show the purpose of trip.
Would someone experienced or someone went through the same please shed a light if the above is only required or any other form/s as well?
My other question and just wana confirm. Do I need to fill out these forms online, take appointment, and then bring all the forms/my passport/papers etc. with myself on the date of interview, right?

On a related note, we are going for a wedding so is the invitation card enough or do we require a letter from our relatives and what would be the contents of the letter.

When we drove down (through Rainbow bridge), invitation card was sufficient.
Great! Thanks for the info. I checked the link and we have to fill out both 157 and 156 forms.
Another question, which I forgot to ask in my initial inquiry (hope I am not being a pest).
I am planning to go and visit States with my wife. Do we need to fill two forms each for DS-156 and 157 - one for myself and another for my wife?
