Not so sure if this form is a must for new immigrants below 16 yrs. to
produce for school admissions.
The immunization / vaccination form can be found at this link. This is something new to me and good to have discovered it before returning.

These forms , cards and books for maintaining of Health Records are available with the Doctors all over the world. Those who are frequent travelers do carry such books and also maintain it for the examination. In particular they look for a YELLOW card with 'Yellow Fever Vaccination Record" for those who did travel through those infected areas.
If you insist on getting one, the Family Physician will certainly oblige you with one.
The format of the card is reproduced here and resembles the one you have provided too. They also could be in the form of a Book, the size of your Passport.
Some of the other vaccinations that you have not gotten yet, can be obtained here through the schools for the children when they get announced, either through the school system or through any clinics here in Canada and most of them are FREE. If at that time you do have the booklet or the Card, they will update it for you with the batch number of the vaccine that the child was inoculated with.
For other Vaccinations such as TWINRiX etc., they are optional and cost a lot. And a few others you must consult your Family Doctor for its suitability.
You should consult the local School Board requirements at each location for what it is that they will need, prior to enrollment. Or carry what ever that you currently have been immunized with and get the balance completed here.

We are planning to move from India. Can you please let me know the exact format or can it be written in the doctors letter head, signed and sealed? I just know all the Vaccination given to my kids and have maintained a record, but its not a proof. Please help me to make one.
