How much time does it take to apply for ON Health Card? Do all the family members need to come in person?

If you mean how long it takes at the office it depends on number of people at the time of your visit. If you mean when you can apply, you can apply after landing as soon as you have applied for SIN and you have leased or bought a house in your name. But you can start using the health card only after 3 months after landing.
Yes all family members have to go in person and they will take your pictures during the application process.
I think, it doesn't matter how many weeks it takes to reach you OHIP card but you are covered by OHIP as you complete 3 months living in Ontario. If you finish 3 months in Ontario, you can use OHIP application confirmation to use OHIP services. If card delays after completing 3 months and mean time any services you have paid cash, would be Reimburse by OHIP when you claim. You can apply for OHIP once you have required documents (advisable after 2months after landing). In Toronto normal time, OHIP card arrives with in 2-6 weeks.
