You need to live in Canada for 3 years not 4 years for citizenship

The Health Card is given by the Provincial Govt. where as Citizenship by the Federal Govt.
As regards travelling by car, they do take a pciture of your licence plates everytime you exit & enter the country. At times they even enter your info. in their records and you won't even know. I've experienced it, when entering Canada by car, sometimes they double check when you had left the country by pulling up your records. However this is mainly for duty-free allowance purpose. But the fact remains that they have the means to do it.

If he/she filled out all information correctly and it was the clerk's mistake, I don't think they have anything to worry about. On the other hand, if they lied on their application, they may land themselves in trouble at a later point.
Same goes for citizenship. You'll hear stories on how someone stayed in the US and did not complete the mandatory 3 yrs. but got citizenship by not mentioning this on the application (which is effectively lying). What these people don't realize is that at some point in time, they can run into some very serious issues and the misinformation will come back to bite them in a way they never expected.
