Hi, I had few(whole lot of story) issues with my health card two years back and they cancelled my health cards. I proceeded legally with an attorney and proved myself strong and got the approval for my health cards. They sent me an letter that from Feb 2006 my health cards are effective and i will be getting my cards in two weeks. I got the letter two month back but i did not get the cards still.
Q 1: Do I have to contact the health main office in Kingston or my local health office to get the cards?
Q 2: When they revoked my health cards, I did not had health insurance for two years. last year may I was travelling in US and got sick (kidney stone) so I had to go to an emergency. They treated my on my brother's (lives in US) promise and my brother still pays my medical bills every month. Eventually i have to give him back. Its about 4500 US dollars. Is there any way I can claim that amount with CA health? because health dept told me that my health cards are effective from Feb 2006, and i got sick on may 2006.?
Q 3: I am waiting for my Citizenship Test any time. do I have to show them my health cards at that time? I applied my citizenship with my Passport not with health cards for identification.?

You have to call their call center about your card and you can ask them about your claim too. See the section "for more information" at http://www.health.gov.on.ca/english/public/program/ohip/ohip_mn.html
Health card is not needed for citizenship though It is accepted as photo ID when going for citizenship test.
