I am trying to apply for PAN Card( Indian )
My questions are
for identity- I can give Canadian Passport
for address- Canadian passport does not carry Address.
My option are to go to indian consulate & get BANK satatement attested by Embassy. Has anybody applied for PAN card from Toronto. Otherwise, even my driving license has my Address. Does indian authorities accept Driving license copy after attestation by Indian consulate

Yes. It is very straight forward & simple. My wife got her PAN card in Mail. Hats off to efficient work from Indian BABUDOM.
Procedure
1- go to NDSL website for applying to New PAN
2- Fill Out Form
3- Take a Print Out
4- Paste 2 Photographs
5- Make a Indians rupees DD by scotia bank
6- Send Form by Canada Post to Pune Address
7- Get your Card delivered to your address by Courier. ( in 2 weeks time)

What did u use for address proof ? got attested frm indian consulate, how ?

which form of the link for nRI pan ?

I applied through the website pancardnri.com.

Sent in application after christmas 2010.
Got PAN Card in mail in the first week of March 2011.

Paid $57 US.

They do all the attestation etc...

Once you pay them, they send you email with clear directions.
After application they send you a coupon # to check status.

If you are not in a great hurry, you may try this.
