Hello CD's,
The application stipulates two pieces of Identification showing child's name and date of birth - one of which must have a photo on it.
Now in their example for school going child, following have been indicated:
- School Record
- Health Insurance Card
Is it possible to provide copy of Indian passport as one of the Photo Id's? Not sure if this applies only for a child who is under school age.
Does anybody have the experience of obtaining a copy from School Record as I have never seen any piece of ID Card provided by the School.
So in essence what might be the second piece of Photo ID for a minor apart from PR Card?
You replies in this regard will be highly appreciated.

You can apply for provincial ID card depending on province you like. It is a piece of card with photo and date of birth of minor. Check with your province you live, whether they have this system.

FYI., Provincial ID is issued to person of 16 years or older.
This is considered to be a substitute piece of ID for a person not having Driving License.
If a person has a driving license then it needs to be surrendered in order to apply for Provincial Id.
In short, a person of 16 years or more is only entitled to one of the ID's.

schools will issue a student status report which contains the name and date of birth and you can provide a latest progress report along with that. Just give a call to the school and they they will give you the prints out, this will fulfill the school record requirement.
