We did all that right from here in Canada. ICICI bank sent us the account opening form and told us all the supporting documents that they need and we sent it to them. We do our fixed account and all Indian banking from here. They even suggested to us how we can transfer NRO funds with minimum tax payment.
Try contacting them to whichever place you want to open your account. We did all this in Hyderabad. Though State Bank of India was pretty easy in the beginning but its difficult now.

Do you need any in one in Hyderabad (for eg) to open this account?
If you do not have any relative in India, which address did you provide?
How much time it takes to open this account?
What documents are required to open this account?
How much time it takes to open such account?
Which a/c is this? FCNR / NRO / NRE?

When I visited ICICI in TO, they just told me that I will have to contact somebody in Hyderabad.
So I went online to their website and filled in the application and one of their representative called me and it was little difficult due to time difference and the way banks in India work but finally, we were able to open our account.
I opened an NRI account and the address I provided was the one on my passport(Indian address , which is not valid anymore) but my mailing address was my canadian address.
It takes a little time but it also depends on how efficent your bank representative is. If he is smart, it doesn't take more than a month. Once you have one account opened, then the rest of things are easy.
I have both NRO and NRI and house loan account with them.
Hope this helps. Try ICICI - they are more online friendly than any other Indian bank.

If I am nit mistaken, you have to send a whole bunch of supporting documents to the bank in India by post.....along with signed forms..... I had wanted to do this, but then got too worried about identity theft, misuse of all such documents by a person I don't know or can't see or can't verify..... So I had waited till I visited India and opened the accounts in person....
Offcourse, each person has his/her comfort level....
