My children were born in Thailand via surrogacy and were issued Canadian citizenship certificates at the embassy there. We recently moved back to Canada and are applying for new certificates as their last names have recently been changed.
Do I still need to complete the section for the "other parent" (the Thai surrogate)? I do have a waiver that the surrogate signed to waive their parental rights and we submitted that along with our application and left the "Other parent" part blank. Will I need to submit that waiver again with my application?

I do understand all of the trials and tribulations that you both went through and the legal hoops and loops that you both had to jump through to start a family of your own. Nevertheless, it sure will be heart breaking to the Birth Mother to part with two lovely kids.
I would have easily asked you to leave it blank. But that is not appropriate. You must provide the same waiver once more to satisfy the LEGAL requirements to complete the paper work. Hope that it will be for one last time.
That is a new field in its own merit, and needs further study to see how the other jurisdictions are handling it.
