Confusion for filling up form additional family info for my wife.
Dear Friends,
I am in process to file application for permanent residence visa under skilled category for canada.
As per the guidelines of CIC, My wife has to fill up the Additional family information form.
Now I am getting confuse while filling up the Additional family information 5406 form for my wife
1. Applicant Name : what should I mention ? either myself or I have to write my wife’s name?
If I mention my name as applicant then what should write in
2 Spouse : should I mention my name or I have to write my wife’s name?
In the same section A father & mother details are to be given, so now what to mention in those column too.

My Name is Ashvin Ravindrabhai
Surname is Desai
Then I have to write
In column
Family name : Desai
And in column
Given Name : Ashvin Ravindrabhai
If I am wrong pl. correct me.

At Present I m working as sr. assistant in Milk product Dairy in Co operative department.
What should I mention as occupation (NOC) out of ..
1212 Supervisors, Finance and Insurance Clerks
1211 Supervisors, General Office and Administrative Support Clerks
1111 Financial Auditors and Accountants
or other if it does not find in above.

You have to fill out the additional family for you and your wife. For the form that you fill for your wife, write your wife's name as the principal applicant. It does not matter although she is not one because you have earlier filled the application form IMM 008 in your name.

can you be your own spouse?
I hope my answer in no. 1 clears out whose name you should when you write down your wife's name as the principal applicant in 5406.

Your given name is Ashvin. So I am not sure you would want to / have to include your fathers name. But like i wrote, I am not sure.

Check the nature of duties you perform and compare witht he ones listed in the NOC. Write the one with the closest match. Do not be too ambiguous.

I have change the information as you said but still I want to ask you one thing,
as I m not clear yet.
Q. While filling additional family info for my wife I would show her name as principle applicant then in next column, name of spouse, again I have to show her name ?
(pl. note I am the principle applicant).

Your name should be in the spouse row.
