Questions while filling Immigration form

Hi,
I am applying for Permanent Residence in Canada under skilled worker. While filling form IMM0008 (Application for Permanent Residence in CANADA), I have many questions. I request you to clarify it.
Right now I am working in USA on H1 visa. I will be here till June 2005 and later I will be relocating to India.

Here is my Questions:

1. Which address (India/USA), I should fill for mailing & residential address?
2. Where do you intend to live in Canada? Is there any specific city/town to mention here?
3. I found in the checklist the following,
If you are applying as a Federal Skilled Worker, use column F.
If you are applying as a Provincial Nominee, use column P.
If you are applying as a Quebec-Selected Applicant, use column Q.
I am applying as a skilled worker; I hope I should use “Federal Skilled Worker” option. Could any one tell me what is other two?

4. I found one option “You’re Identity Card, if any…” in the application. Can I use my USA Driving license for this option?

1. give yr mailing add
2. i.e yr current add where u r residing. coz u will have to give yr
detailed residence history of the countries where u have lived more than 6 months. we gave like 10 addresses since the age of 18
3.give a plkce where u want to go to. i wld suggest a small plce
but dont worry coz u dont have to go and settle there. i will suggest nova
scotia they r not getting many immigrants and have launched this campaing to help[ new settlers. we got alot of help,
4.dont go for quebec unless u know french.and alkso it is tiugher to go there then if u want to go there u can once u have landed
5. skilled worker form sorry i cantr recal the #
6 it will cost u alittle more but go through a law firm i suggest canadavisa.com the name of the lawyer is cambell they r good and helped us all the way. they r inmontreal. i was so glad i went with them worth every penny . they charged us i think 1500$
