This refers to the Immigration form IMM 5406(Additional Family Information Form).
According to Instructions from the Checklist,this form has to be filled by me(Applicant) and also by my wife.

If we fill the forms in the following way there arises confusions,please clarify.
In my form if I include my name(Applicant) my wife name,my parents & also my brothers & sisters names.
My wife in her form if she includes my name(Applicant),her name & also her parents
& brothers & sisters names.
Then there arises a question that how will any one who looks at these forms know which names(parents,brothers & sisters) correspond to me & which to my wife.

Also it is required to write all names in our native language as well.There is not enough space to write names in english & also in native language.
Please clarify these two issues.

This part of the application is really confusion one.
If you notice. In the top, you only fill the applicant section & leave empty the spouse one.
In another simalr form for your wife's . The form shouldl have empty space in the Applicant colown . She shouldl only be filling her name on the applicant spouse's name section.

On your second query, I guess you should be filling the whole form with Fillable PDF format only. On the website of CHC that form is available. So the english language words will take very less space

After taking the printout ,you can use Black pilot pen to fill the names in your native language.

1. What I did was take up the printout of the forms.
2. Then fill all the forms with pencil
3. Then use that information & fill the downloaded forms from CHC site in the computer. Please note you cannot save the softcopy of the filled forms as one required full version of Acrobat reader to do so.
4. Just take printout of the forms.
