what is procedure to start regular E.I after Sickness benefits

HI I have applied for E.I (Sickness) and it is only paid up to 15 weeks. Service Canada representative told me you will not get more Sickness benefits, you should have to apply for Social Assistance. But I would like to start my regular E.I so will you please tell me what is procedure to start my regular E.I after Sickness benefits??

Do you have a job to return to? if not, did you quit/fired/laid off etc?
