Centre Island & 100 islands are two distinct places. Here's how to get there
Centre Island - You have to take the Ferry from Queens Quay. It's at the foot of Bay Street right behind the Westin Harbour Castle Hotel
Tip: If you need to park around there, continue East past the hotel, on your left is a LCBO outlet. Turn into the street besides the LCBO. There's a big parking lot for $5/day.
1000 islands - Drive East on 401. Around Kingston (approx. 250 kms from Toronto), you'll see signs for Gananoque, where the 1000 island are located.

To continue on the 1000 island portion:-
You can exit at Gananoque and take the road all the way to the river (St Lawrence). Signs will show you where you can take a boat ride to see the islands. They have two types, the 3 hr and 1.5 hrs ride. 3 hrs is a bit boring for the children.
Another way to see the 1000 Islands is to drive the 1000 Island parkway. Right after the Gananoque exit there is an exit marked "1000 Island Parkway". It is about 50 kms long (I think) and reconnects with the 401 further east at Mallorytown. It has plenty of lookout spots and picnic areas. You get to see the islands from terra ferma.
Good day trip and family picnic type activity. Recommended.
