Dear Desis,
OHIP sent us a form to confirm the name of our newborn baby. 3 fields are there. Besides first name and last name fields, there is a centre field for child's second name (not indicated as middle name).
We did put a middle name for the baby in the birth registration form (statement of live birth).
So now I am wondering if I can put the middle name as second name in OHIP form or leave it blank? Are second name/middle name(initial) both the same?

Dear abhicanada,
There is a phone number provided by OHIP for you to get in touch with and find out what are the details that they want in the columns that you fill in their application form.
You have given the child a MIDDLE name. Ask them if it is OK to insert the same as a SECOND name. If they say it is O.K., then, that makes everyone happy. Otherwise ask them if you can leave it blank.
What are the kindly gestures that you do to celebrate when a child is born? Do you give out Cigars or Ladus? OR do both?
