The fee is $75.

I submitted the following alongwith the application:
1. My employment letter
2. Declaration that I will be supporting them for their entire stay (invitation letter).
5. Copy of the passport including the original visa stamp page.
3. Their medical insurance for the next 6 months
4. Return ticket.

You have a choice to even fillup, apply, and submit the form online without sending it via. snail mail. You can attach the supporting documents as well.

P.S. No medical exam was requested.

Same happens to my friend's mother. He had applied for 3 month extension and got 2 years VISA. She had regular 6 month visitor visa (not supervisa). I hope it will work for you too.

The supervisa is a new visa , so i guess it will take time for immigration guys to get used to giving out more than 6 months stamping..
