I am a new PR landed on 27 April. I received the PR card of my wife on 26 May, but with an error, the picture on my wife's card is that of my son. I've called the Call Center but each time I get a different reply on returning the card and getting a new one. I've searched the CIC web site, but it does not list any procedure for returing wrong PR cards and getting new ones.
Someone, please advice on the procedure to solve this problem.

You'll have to send in the defective card with a note confirming your conversation with the person from the Call Centre.
The rules talk about 'destroyed card' - this card has been destroyed by CIC's incompetence. [I am assuming you completed the forms correctly.
Make sure you have copies [preferably colour]. It's better to complete the forms again just to provide CIC with accurate info. DO NOT PAY ANY FEE.
I'm just curious to know whether whose picture is on your son's PR card.
If you have any problems contact me directly. I'd like to publicise this gaffe by CIC [with your consent, of course]. Take a photo of the card with a newspaper for that day in one hand and the PR card in the other. Please see my contacts below:

Thanks Mr. Massood for your time and advice. I called the Call Centre several times and finally got some response. They mailed me an application to reissue the PR card and return the one with mistakes. I did that yesterday 31st May, and now it will take another 3-4 weeks to get a new one.
I've kept a copy of the PR card (black and white) for record. Unfortunately I did not read your reply before sending the application & card back to PR card centre, I would have taken a picture as advised by you.
I would take some more time of yours, and give you the background as to how my son's picture ended up on my wife's card. Actually the client ID for my wife and son turned out to be the same. We did not discover this untill three weeks after landing in Canada. When I called the call centre to check on the PR card status, we realized that these IDs were same. Following that we were given a new ID for my son, but I believe by then my wife's PR card was already made and mailed.
Now I am waiting for my son's card to come and most probably it will have my wife's picture on it. So much for the most secured and tamper proof card in Canada.

Today I received my and my son's PR cards without any errors. My wife's card will be reissued within 3-4 weeks. Hope it will be free of any errors this time.
