Need some advice here wrt my parents' sponsorship application process ... Have been asked by CIC to furnish documents like birth certificate , marriage certificate , passports etc .
My father's last name ( family name ) is not there on his passport or his High School certificate or his driving license or PAN card .... only the first & middle name are there .... for eg Anup Lal ... My mother's name is complete as spouse on his passport .
But in all the other documents viz their marriage certificate , my birth certificate , my mother's passport ( spousal name ) , my own passport ( father's name ) full name is present for eg .. Anup Lal Gupta ....
Does this appear to be a major inconsistency .. ? If yes , should we submit the application to CIC as is and await their response before taking corrective active or should take corrective action prior to sending the application ?
If we decide to take prior corrective action , what is the best thing we can do ?
a. Get the Passport ammended so as to include the last name as well .... Would this be a minor correction or a name change process as he is already traveled abroad else where on that Passport ? .. I m assuming for a name change process , one has to advertise in the newspaper for 30 days or something ....
b. Get an affidavit from them as well as a Gazzetted Officer that Anup Lal & Anup Lal Gupta are the same person ? Provide a wedding photograph as well .
Wondering if we go ahead with option a .. will it be a long process of advertising in the news paper and requesting a name change OR a short process of endorsement since only the last name is to be added ....

Go for the option b. I had the same issue, where the passport and other certificate names didn't match (unfortunately, there's no tradition of adding the last name to the name in India). So, I provided a handful of affidavits duly signed by my Dad and Mom each, and the application got approved with the same.
You don't need to go for the option a. at all.

Did CIC ask you for this discrepancy during the Application process or did you do it pre emptively as I am thinking of doing .
I must ask you how many affidavits and roughly what did you include in these ... I can think of the following ones :
A. My father states that his name per High School certificate and passport is first name & middle name but his family name is last name , hence his full name is first - middle - last name . And that he has been married to my mother per marriage dated ... per certificate ....
B. My mother also gives a similar affidavit .....
C. A noted Gazzetted Officer gives a certificate that he has known them as per their complete names for so many decades and that both the people in the certificate & the passport are the same (in case of my father )
D. Plus may be some wedding pics ...
Could you advise if there s anything else that can be done as well ... Also .. which name should I give in the Application for my father .... the name in the passport ( for eg Anup Lal ) OR the one in other certificates viz marriage certificate , my birth certificate ( eg Anup Lal Gupta ) ??
Appreciate your advice since you have undergone this ....

I am yet to have a look at my affidavits, so can't tell you for sure. But, here's what I did:
1. Affidavit 1 for name: Something like this, "I hereby state that my name is ABC. <former name> and <present name> is one person.
2. Marriage affidavit 2: From both, mom and dad. This includes the date and place of marriage.
3. Wedding pictures are not required
4. Date of birth affidavit from each of them: Contains date and place of birth.
5. Relationship affidavit: That they are your parents. This will also include date and place of birth as well as your name.
6. The name which he goes by currently should be on the application, and which is mentioned on the passport as that's his legal name now.
7. Newspaper advertisement is not required for any name of change, etc.
All affidavits must be duly signed and notarized.
Hope this helps. I am currently away from home so don't have the affidavit copies with me, but if in the meantime I find one somehow, I will send you the samples.

Update & advice needed .
( My father's surname is not present in Passport but appears at marriage certificate & my birth certificate )
My father got the affidavit made yesterday in India ... which goes similar to this :
I Anup Lal hereby state that my surname is Gupta and my full name is Anup Lal Gupta , initialed AL Gupta . The 3 names - Anup Lal , Anup Lal Gupta and AL Gupta refer to one and the same person .
Now the qs :
a. This was done on a paper (not a stamp paper) and duly stamped by the Notary as they do here viz , 'Signed before me on the 24th day of May 2012' - Does this need to be on a stamp paper as the norm in India ?
b. On the Application form , the Names are to be filled in 2-3 places , Last Name , Given Name ............... In only one of these places it states that Last Name & Given Name should be exactly as in Passport ... At the other 2 places such is not mentioned ....... And yet at a place it is mentioned to provide any other name that you have used ......
My dilemma : My fathe's name in the Passport is (say) Anup Lal ..... So in the place where it mentions to provide the names as per Passport , we are not entering the Surname .......
But in other places where it asks for Full Name ... Last Name ... Given Name ..... Should we enter Gupta Anup Lal OR leave the surname blank ..........
Will it be wrong to provide the surname at places other than where it says should be same as Passport ? We are providing a declaration that the surname is Sharma .....
I was thinking of leaving the Last Name blank where it says per the Passport and enter the surname at other places .........
Please advise .....
