Dear Masood bhai,
I am a convention refugee but what happened that when I claimed refugee status CIC followed my family name written in passport which had spelling mistakes. I requested them to put a note that these are not my original spellings and also told them my right spellings, but they followed the wrong spellings in order to go with the name written in the passport suggesting that later I can correct it.
Another wrong thing happened was that IRB added another alphabet in my name by themselves of course by an error. I called them and mentioned that they wrote it incorrect, they are saying first correct it from CIC and send them the proof of correction and they will correct it. I moved from Montreal as wanted to start job. The main problem arose when my social worker referred me for the training for security almost 2months ago and I passed the test but problem became the spellings of my name as my health card is issued by the name given by IRB, my G1 license is only in the original name only, my Sin Card & Work permit is of the spellings which CIC has, so the management of that training centre were amazed to see that and decided not to let me proceed with the training and job till I get it corrected from Immigration. Well, I was very disappointed as I am very much in need of a job as have to pay a big big amount for my permanent resident application fee. Lately, I corrected my spellings from the Consulate of Pakistan in my passport and called CIC to seek advice what to do?
CIC sent me forms which I filled and sent with all my proofs I had to support my application to the CIC office in Scarborough, they sent my application back suggesting to fill another form “Amendment Kit”. I called CIC again and asked what to do now , they advised me that I am not a permanent resident so I don’t need to fill any form and just send a letter and proofs to CIC stating all my condition. I did so, you know what happened, again my all papers came back suggesting to call CIC.
Finally, today I called CIC again that what should I do coz already my time is wasted a lot and I am unable to work and undergoing mental stress and financial constraints as well. So today they suggested me to fill the work permit form again and to send the case processing centre in Alberta for issuing a new work permit in the correct name along with the proofs and old work permit which is not expired yet.
Main question I wanted to inquire is that now I am about to fill the forms and to send them for the permanent residence application for me and my family, so what name should I put in that ? Can I write my original name and should send them the proofs like my corrected passport/birth certificate/marriage certificate/educational credentials etc?
Kindly suggest me accordingly
I did not read your whole post. I'm going with the title [subject] of the post. If the name is misspelt, please contact CIC asap or else you'll have huge head-aches later.

Dear Mr.Masood,
Although you didn't read the whole issue I described, yet I am thankful to you for replying, I have contacted CIC but they were confused too, I am amazed coz this type of issue would probably be not the first time with them, anyway, thanks once again.

Dear Mr. Masood,
Thanks for being kind once again suggesting me to write my original name in the permanent resident application form alongwith the proofs and stating all the steps I took to eliminate my wrong spellings to immigration. I keep copy of every paper I send to immigration as a record.
Hoping the rapid solution of this issue.
Thanks
