Can anyone know, What is difference between long form and short form of birth certificate?

The short form is just like a card. It has the name, place of birth and date of birth.
The long form is like a scroll and has parents' name as well.
It is similar to the format of Indian birth certificates.
Depends on what you need it for.

Long Form is the replica of application you filled.
Short form birth certificate is mostly used for passport, school admission.
It is better to have a copy of long form also.

The Passport Office is now asking for the long form birth certificate if you are Sri Lankan or Indian ethnicity.
