Canadian Immigration as a skilled worker

Hi to everyone here.
I have a few questions for this forum.
I am currently based in NY, USA.

[1.] I am contemplating applying for Canadian immigration under the skilled workers category.
I am a Masters degree holder and am a marketer by profession. I have 7 years experience. My wife, also a Masters, is an IT professional and has 4 years experience. Between the two of us, we can muster more than the 67 points required.
Who in yr view, shld the 'principal/main' applicant be ?

[2.] I have a brother in India who has a Bachelors in Commerce and a Masters equivalent diploma from a premier, autonomous institute directly under the Govt. of India.
Can his name be added to the application as well ?

[3.] What are the different stages of the application process ? How do they follow one another ? Here I mean medical, interview, ROFL (all words heard here on this site).... etc. etc.

[3.] Where should I make my application? Buffalo, NY or New Delhi ?

[5.] Finally and most importantly .... what is the 'total' waiting time ?

1. Whoever can produce higher IELTS score
2. No
3. Application, acknowledgement, medical/interview, RPRF, visas
4. NY, if at the time you entered USA, you had at least one year visa [not visitor]
5. NY= approx. 18 months; Delhi= approx. 26 months

A re-joinder to what I had earlier posted ....

-- [1.] Is IELTS mandatory? For both? I have a good TOEFL score and published articles in national Indian dailies. Will that do in lieu thereof?

-- [2.] Would u advise that my brother makes a separate immigration application?

-- [3.] Can I make an application now and submit my medical in, say, Feb 2004. ( Does one need to submit the medical with the initial application or does one receive a 'go-ahead' later ?) This, 'cos I am expecting my first child in Dec-Jan this yr.

-- [4.] Will any of the applicants be needed in NY, post application and medical ? ( Can I proceed to India, giving a forwarding US address?)
Also, what is a interview waiver ?

1. IELTS for principal applicant only. Looks like you.
2. Yes, if he qualifies
3. The embassy will tell you when to do medicals. It's several months after application.
4. Only if there's an interview. You can be in India, butbeware some of my clients' had their files transferred to their India after they moved.

Interview waiver = interview is waived by the embassy [no interview]

I am a trifle confused

--[1.] Is IELTS a must ? ( 'cos the Canadian immigration site does not explicitly say so). Can a TOEFL score be given instead ?

--[2.] How can I preserve the American citizenship of my child while I bring him under the Canadian Social Security net ( my prime concern is health/medical insurance) ? :confused:

--[3.] The fine print for Family Class Immigration says that parents can be brought over to Canada provided they are not an 'excessive' burden on the Canadian health system.
In practice, can parents be refused entry for minor, age related health ailments too ? :confused:

1. IELTS is not a must if you can convince immigration of your fluency in the english language. You will do this by making extensive written submissions.

2. Canada recognised dual nationality. As far as Canada is concerned, it's not a problem. ... But your child will only be a PR not Canadaian citizen. So, the issue is superfluous.

3. if you sponsor your parents, their application can be refused if they have a serious medical condition.

What kind of documentation are you referring to? Can you please provide an example or two. what if the applicant just sent a letter seeking exemption to this requirement given his/her command over the language, at the time of the application. Application number has already been received.
