IELTS results are valid for two years only, I have to apply from Islamabad, 64months waiting time in Simplified process, after 64 months do I have to take IELTS again or what ? If I wait for the 64 months and do not take IELTS , how can I claim my language proficiency in application form. Also I Have 5 years USA Visit Visa, can I apply from USA for Permanent Residence.

Please have all of the required qualifying papers PRIOR to the time of FIRST application. If you do get the required POINTS, then your application is A.O.K.
Then if they require the same once again they will let you know. If it happens that you have to go through IELTS, so be it.

No. You cannot apply for permanent residence from a country outside of the first application, unless you are employed there PROFITABLY. Hence you will have to go back and carry out the required process from the country that you first filed from. (Country of Residence). If your stay gets to be long and extended, then you may also require a Police Clearance from that new country.
That is what I know for sure from my last reading of the Rules.
