Goods to follow list ?

Is there a standard "goods to follow" form where we have to fill out the details for canadian customs or is it something that we just write up ?

If you could point me to a sample "goods to follow" list that would be great.

There is no standard form. Just write up a list [like a shopping list].

You can call the document: "GOODS TO FOLLOW"

Sign it.

Date it.
