I am facing same problem. My kids and my possport do not have surnames filled.
I applied for Immigration in Aug 2004 to CHC, London.
I got AOR in Oct, 2004. I am expecting another year before this issue becomes crtical and would like to be ready by than.
Like you I am residing in UAE and moved for migration as explained above.
I request your kindly advice me in detail, if possible, procedure or steps for adding surname in the passport.
Also clarify me, will that have any effect on current residence visa which is in the passport ?

Try overtaking child's imagination
It arose because of my Son's name in the birth certificate. Since I was in UAE, the procedure I followed was, had an advt placed in India, a similar ad placed in the UAE, along with copies of the ad and the originals, gave the passport and appl form to the consulate for amendment. They cancelled the earlier passport and issued a new one. Let me mention here that since my son's passport was issued in UAE, it took me a month's time. I presume it would take longer from a country while the passport is issued in india
