Pls ensure that all the passports of the applicant and dependents carry the surname. I am one of the effected few. The process, for Indians, is an ad be placed in India(preferably in the newspaper of the region where the passport is issued from or the permanent resident address) and in the country of current residence. Originals have to be produced to the Indian Consul authorities along with an affidavit for the change of name. Once the affidavit is attested, a new passport form along with the old passport needs to be submitted attaching the original affidavit. The authorities are insisting on the surname on the passport as I presume, the PNR card will be issued based on this.
I suggest that those with, passports without surnames do the needful so that valuable time need not be lost towards the fag end of the process

good luck. I changed my name from Indian Passport without these hassels. When I arrived here I got the Health Card and Driver's Lic in my short name. When I applied for Canadian citizenship I used the same short name and provided copy of Driver's Lic.
