My passport contains my maiden name &surname. Hence my landing papers and PR card too.
However, I have my SIN card in married surname(after producing the marriage certificate).

I learnt about the required documents to change my surname in passport and hence in PR card. Among them it quotes,
1) Declaration for non-acquisition of foreign nationality.
2) Affidavit by applicant mentioning about change in name.

How do I get these done?
I tried to call and write to the toronto office/mail id , but in vain.
Anyone can guide me?Should I be going in person to the office and getting the details?
