Hi

I recently got a birth certificate made for my baby that included parents name. (I got this made as i would be applying for a PIO card for my baby) The certificate shows the mother's maiden surname and not the surname after marriage. Is this an error on the part of vital statistics agency?
If i submit this certificate with the indian embassy will there be a problem as my indian passport shows my surname after marriage and not my maiden surname.

Thanks for replying

Did you provide the details in the application form for them to inset it into the Baby's Birth Certificate. See How they work it into the Certificate.

If you think it should not be there and want it differently, then get a second form and inform them by filling the way you want it in the application. That way you will get the birth certificate written in the proper manner you want. Fill it like the way you want.

In short, leave your maiden surname out completely. Or fill in once again your married surname. Is there a guide that comes with it and explains how to fill it?

Here is the application at the bottom for you to read and understand. You should have given the SUBJECT's LAST name at BIRTH as the one you want in the certificate. (Not YOUR maiden surname.) Some Confucious generated this form.

Freddie.

Thanks Freddie
The form of B.C vital stat is very different from ontario. The had asked for both mother's maiden surname as well as married surname. I will send the certificate back and see what they have to say.
Thanks for your help

You can submit it with a copy of your marriage certificate and a letter explaining the same. You shouldn't have any issue getting PIO.
