I know it is mind boggling when you see so many pages and what to do. Here is the list of things that you need to do.
You are traveling to Canada on a travel document. That only lets you in, into Canada.
This is the application Form that you have to fill, after you reach Canada and settle down in a place where THEY can send YOU the NEXT PR Card. Please make sure that you stay there till you receive that card or ask some to forward it to you if you change your address. Other wise you will loose it a second time.
Since you did not receive it the first time and don't know where it went, you will have to swear that you did not receive it. It is called Solemn Declaration and you do this in front of a Notary Public. He attests it. Which means that you said "that you did not receive it" and sign that document in his office and he is the Witness to that document and puts his stamp or "Chapa" on it.

After you fill your application and get everything ready, here is the check list to tick all of the materials that has to get into the mail. You please fill out the same and tick against the numbers and make sure all of it went inside. Attach it to the application. Here is the guide. I know it is a long set of instructions. Read it once or twice and get to know it.

NOW here are the answers to your questions:
1) Very simple. You fill out the application forms, after you land in Canada. ( I only suggested that you have them ready, if you have time there in India to down load and print and do the filling) You can do that after you settle down in a place here in Canada. (Kitchener, ONT) Not at the air port.
2). Ask for the forms at the air port at the Immigration check point. You do it after you reach Canada and settle down comfortably in a place. (At Kitchener)
3) I am not sure of any place in Kitchener where you can get such help. But if you have a phone that you can use to call Toronto, then, I will give you a link and you can talk with them and see if they will help you over the phone.
