It means, fill out the application form (that's already there on the website)completely and print 4 copies when submitting to BLS.
I am assuming the application form will become available once you initiate the online registration process (although there is an option to download the application independently).

The ON LINE form that you will fill in to get started will be just a short one.
The PRINTABLE form will have 29 items to fill and complete. They call it the PER FORMA Application.

It needs to get filled in FOUR DIFFERENT Copies in BLACK INK and you need to carry the same and submit to the local BLS offices and get the process completed at that location satisfactorily to them.
